491 Over 90 years of age is an unfavorable prognostic factor of cutaneous squamous cell carcinoma

Background: Surgery is the golden standard for treating cutaneous squamous cell carcinoma (cSCC). However, dementia and other complications can render surgery challenging in elderly patients. In this study, we analyzed clinical characteristics of patients over 90 years old discussed appropriate treatment options. Methods: This retrospective study 294 with cSCC who received at Kagoshima Medical Center from October 2014 to September 2021. Results: The “over 90-year-old” group (90<G) had 69 (23.5%; males: 27, 39.1%; females: 42, 60.9%). “90-year-old or under” (90≥G) 225 (76.5%; 129, 57.3%; 96, 42.7%) (p<0.009). Tumor diameters were slightly larger 90<G (mean: 21.67 mm; range 5–90 mm) than 90≥G 18.83 2–120 (p=0.128). average tumor thickness 5.18 5.69 mm, respectively (p=0.422). 90<G, cells significantly invaded beyond fat (p=0.0017), rate positive margins was higher that (p<0.021). Kaplan–Meier survival curves relapse-free (RFS) two groups showed recurrence risk (p<0.005, HR 3.24, CI 95% ratio: 1.483–7.076). Age (HR=3.918, CI: 1.714–8.958, p=0.0012) degree differentiation (well/moderate: HR=3.296, 1.376–7895, p=0.007; well/poor: HR=4.969, 1.3420–18.4, p= 0.016) independently correlated RFS. Conclusion: old, tends invade deeply, surgical margins, have a high risk. results may inform management old.
